Health information exchanges (HIEs) are networks of electronic health information contributed by various providers. By seeing records of past care received at other locations in an HIE, providers can make more informed decisions about care plans and avoid duplicative or unnecessary treatment. We participate in several HIEs, including Atrium Health CareConnect and NC Health Connex (click on names to access their sites and opt out forms). You do not have to participate in an HIE to receive care from us, though note that opting out of an HIE does not stop us from using or sharing your information as described in the Notice of Privacy Practices. Visit the HIE sites listed above to learn more about how they share your information. The forms to opt out of participating in the HIEs are posted on each of their sites, as are the “rescind” forms if you later decide you want to re-engage in the HIE. We may update this list from time to time.

Note that we are required to share notification of your admission, discharge, or transfer (often called “ADT” information) from our facilities to your other providers. If you want to opt out of us sharing your ADT information for a particular visit, please alert the staff at the registration desk at the facility where you are being treated.
Atrium Health owns Carolinas HealthCare System ACO, LLC (“ACO”), which is an accountable healthcare organization made up of participating providers known as “ACO participants”. Through the ACO, our patient information is combined with that of other ACO participants so the ACO can perform certain functions on our behalves, such as care coordination and joint utilization and quality assurance activities. The ACO participants have also created an organized system of health care through the ACO that allows us to use and disclose patient information in our respective records systems for permitted treatment, payment, and health care operations purposes. We also engage in an organized healthcare arrangement with caregivers in a clinically integrated setting, such as with our medical staff and care coordinators.
