Our expertise across the surgical spectrum, combined with a collaborative approach and the latest remote and mobile technologies, enables Sanger to constantly improve surgical outcomes.

Recent Highlights

Tailoring intervention to each patient: Our specialized high-risk heart surgery team – which includes a cardiologist, interventional cardiologists, intensivists and cardiac surgeons – continues developing ways to improve outcomes, by following a patient-centered collaborative approach that helps identify which therapeutic interventions are optimal for each patient.

High-risk discharge pathway: We implemented a high-risk discharge pathway in 2022 that includes multidisciplinary coordination and planning, individualized discharge plans and frequent follow-up visits. This pathway has helped reduce readmissions.

Mobile-integrated health: We have embraced mobile-integrated health (MIH) for cardiac surgery patients with dual diagnosis of heart failure. These patients receive a home visit from a paramedicine team, which helps patients who have limited resources.

Innovations

Remote monitoring improves ongoing care: Our remote patient monitoring effort uses biosensors to provide clinicians with data – including heart rate, blood pressure, body weight and steps – that aids decision-making between follow-up visits. We use remote monitoring to continue providing patientcentered care up to 90 days post discharge.

ERAS® fuels improvements: Since launching a cardiac ERAS (Enhanced Recovery After Surgery) in 2022, we have decreased postoperative length of stay and increased mobilization postsurgery. Our post-procedure length of stay is below the STS benchmark.

  • Isolated CABG procedures for a 30-day mortality rate, risk adjusted, show sanger with a 0.79 percent rate comparing favorably to the 2.38 percent benchmark.
  • Isolated CABG procedures for a 30-day readmission rate, risk adjusted shows Sanger has a 5.26 percent rate which is favorable to the 9.11 percent benchmark.
